Definition
When something changes.
The term is usually used in social care in relation to young people who require care and support or young carers reaching adulthood, and the move from children’s social care to adult social care.
The term also refers to people changing their gender from the gender assigned to them at birth to the gender that aligns with their gender identity.
The bigger picture
Talking about transition can be confusing due to the different meanings of the term.
In social care, the term is associated with pathways and processes, and it isn’t a term we’d associate with becoming adults in everyday life.
Saying it differently
Lots of councils use the term ‘preparation for adulthood’ instead of transition.
It’s good to use everyday language where possible, so maybe think about using phrases like ‘making plans’, ‘preparing for changes’ or ‘thinking about the future’.
