Re-assessment

Definition

A further assessment of a person’s needs or finances that a local council may carry out if the person has already had an assessment and a review identifies that the person’s needs or circumstances have changed.

A re-assessment should be a proportionate assessment that considers what is already known about the person. Local councils cannot re-assess a person’s care and support needs without first completing a review and deciding that changes to the person’s care and support plan are necessary.
