Discharge summary

Definition

A report or letter that is sent to a person’s doctor (GP) when they have stayed in hospital, so that their GP knows what has happened and what needs to happen next. The person should also be given a copy. The report should explain why the person was in hospital, what treatment they received, the results of any tests that were done, what changes may have been made to their medication, and any follow-up care that is needed.
