Care records include basic personal details like name, address, date of birth, close relatives and carers, as well as information about the person’s health and social care needs, what is important to them, and what has been agreed about their care and support.
Organisations must make sure records are kept safely, and should ask people if they are happy for their information to be shared with other people and organisations who are involved in their care. People have the right to see their care records and should receive a copy of all assessments and plans.
