Short-term care and support aimed to help people to recover after being in hospital, or to prevent people going in to hospital.
There are four types of intermediate care:
– crisis response – services providing short-term care (up to 48 hours)
– home-based – support from a team of mainly health professionals such as nurses and therapists that people draw on in their own home
– bed-based intermediate care – care and support away from the person’s home, for example, in a community hospital
– reablement – support from a team of social care workers that people draw on in their own home.
Intermediate care is available for a limited time, usually up to six weeks, and is free of charge.
