Transition between health and social care

The NICE Collaborating Centre for Social Care (NCCSC), hosted by SCIE, has been commissioned by NICE to produce guidance on the coordinated transition between health and social care.

They are currently recruiting members for the Guideline Development Group for this topic and welcome the following people:

• social care and health sector professionals,

• people who uses services and carers, academics and

• people in the wider health and social care community.
