TLAP National Personalisation in Health and Care Conference - 1 day to go!

TLAP is hosting its first full day national conference on personalisation in health and care tomorrow. With the Care Bill making its passage through parliament, it's critical we come together to share our work and that of our partners.

The day will be grounded in the experience of people who use services, carers and families and will share what TLAP's partners and others have learnt to date and where we can have the most impact over the next three years.

Topics on the day will cover:
• The commitment to integrated care and support
• Integrated personal budgets for health and care
• Making personalisation work for everyone, including people with complex needs and mental health conditions
• Making it Real in practice
• Supporting Health and Wellbeing Boards
• Information and Advice
• Co-production and equalities will be an important theme running through the day
