Coalition for Collaborative Care: Action for long term conditions

The Coalition for Collaborative care is a group of individuals and organisations across the health, social care and voluntary sector who aim to make person-centred, coordinated care a reality for people living with long-term conditions.

The Coalition is using the House of Care approach to offer local areas evidence, knowledge and outreach advice to establish care planning in an effective local framework - and at the same time recognise the importance of non-medical, community-based activities and resources. This 'more than medicine' approach helps each person develop the knowledge, skills and confidence to manage their condition/s within a supportive community.

The event: Action for long term conditions will take place on March 13th 3-4pm, and is relevant to:clinicians, practitioners, therapists, care managers, people working in primary care and hospitals; other interested parties.

Presenters will include Sue Roberts, Chair of the Year of Care Partnerships, and David Paynton, National Clinical Champion, RCGP Centre for Commissioning.
