The Local Government Chronicle and the Health Service Journal have announced the top fifty care integration pioneers.

In partnership with Nutricia, readers of both titles and social media users identified the 'Care Integration 50' from across local and national government, NHS, third and private sectors.

At a time when there are cuts to services and rising numbers of people with complex needs, it is especially important to have integrated health and social care services which provide efficient seamless care and support to people with care needs and their families.

The model avoids wasted resources, duplication of work and allows for unhindered person-centred care - which is one of the drivers for integration.

Many of the Care Integration 50 candidates are supporters or members of the TLAP consortium.
